Appointment Type

Consultation appointments are made Monday through Friday and are based on your physician’s schedule. The scheduling staff will make every effort to accommodate your schedules. For this appointment you will be asked to fill out a New Patient Assessment. To save time, you may download this form and bring it with you.
Chemotherapy treatment appointments are scheduled between 7:30 am and 4 pm. The last appointment for chemotherapy is 4 pm.

Radiation oncology appointments are scheduled beginning 7 am.

Follow up appointments are scheduled during normal hours.

Make an Appointment

Appointments can be made in person, by phone, or online. The staff works closely with your physician to plan your care. If you need to change or cancel your appointment, please notify the Department.
For The Cancer Center: Appointments may be scheduled via the telephone, 201-996-5900 or on online using the link above. If making an appointment using the phone, the receptionist will forward your call to the appropriate staff member or department. After 5:30 pm, all telephone calls are forwarded to the answering service. If you need to speak to the physician after 5:30 pm, the answering service will notify the physician on call.

What You Need to Bring

Insurance
Once you have the appointment, you may be asked to bring along or forward several pieces of your medical information: your records, laboratory results, X-ray or imaging results, or other test results as requested by a Cancer Center physician.
You will also need to bring your health insurance card, which may include a private carrier, Medicare, or Medicaid. If your health insurance requires you to do so, please bring any referral forms, prescriptions, and co-pay fees that are necessary. You will also need to check if your physician is in your health plan’s network. If your appointment requires pre-certification, please check with our office to see whether our Cancer Center staff must arrange the pre-certification or whether you may do so yourself.

The Cancer Center is located in Hackensack University Medical Plaza, 20 Prospect Avenue, in Suite 400. Radiation Oncology is located in the St. Johns Building of the main hospital.
